PAPERWORK COMPLETION
 
New and Returning Student-Athletes
Be sure to allow about 30-60 minutes to complete filling out your profile. Your completion of the athlete portal for Sports Medicine DOES NOT fulfill the requirements for the Engle Center. Be sure to find the instructions from the Engle Center and complete those separately. The window to complete forms is from May 1 - August 1. The deadline for paperwork completion is AUGUST 1st. If you fail to complete paperwork before this deadline, your participation on your respective team may be delayed.
 
NEW Athletes
Be sure to have the following to successfully complete your athlete portal: Click here for New and Transfer Messiah Sports Medicine ATS Instructions.
  • Your student ID number,
  • PDF copy of your physical (must be completed within six months of participation in NCAA collegiate athletics on campus at Messiah University),
  • PDF copy of your sickle cell lab results*,
  • Emergency contact information,
  • Health insurance information including electronic copies of your primary health insurance card (both front and back),
  • Any miscellaneous documents, such as the ADHD medication form, clearance from a medical condition or injury from the provider, restrictions from a physician, etc.
*This requirement is new in the Fall of 2013. Most children born in the United States since 1994 were tested at birth. Verification may be easily accomplished with review of medical file. If verification cannot be obtained, a new test will need to be performed.

New as of June 5, 2025, PA newborn sickle cell screening no longer meet the NCAA requirement for sickle cell testing. A sickle cell solubility test is required as per NCAA, and isoelectric screening does not fulfill that requirement. All other states’ testing must ensure sickle cell solubility test. Click here for more information. 
 
The NCAA mandates that all first-time college student-athletes receive a physical exam within six months of the start of his/her traditional or non-traditional season. To avoid having to get two physical exams, wait until March 1 before getting your exam.
 
RETURNING Athletes
Since this is a new system, you will need to set up your ATS portal. Click here for Returning Messiah Sports Medicine ATS Instructions.

Be sure to have the following to successfully complete your athlete portal:
  • Your student ID number,
  • Emergency contact information,
  • Health insurance information, including electronic copies of your primary health insurance card (both front and back)
  • Any miscellaneous documents, such as the ADHD medication form, clearance from a medical condition or injury from the provider, restrictions from a physician, etc.
 
Returning athletes DO NOT need a physical from a physician since you’ve already received a physical upon entering college.

If you take any ADHD medication, please be sure to have your prescribing physician fill out the ADHD medication sheet and upload to your health portal as an eFile. Click here for the NCAA Medical exceptions form.
